This week’s in-class activities will allow you to practice challenging and difficult conversations with
patients and/or their support people. To prepare for class, review Chapter 6 in Cho & Cooley, and The
Joint Commission Quick Safety issue 47. You may also wish to review Chapters 3, 4 & 5 in
Cho & Cooley to prepare for this class.
Note: there are two parts to this assignment. In the first part you will reflect on your own experiences
with challenging conversations. In the second part you will choose a case study and prepare some
statements you will use to role-play the case study in class. Both sections need to be completed.
1. Think of a time you had a challenging or difficult conversation with a patient or their support
person. Perhaps you had to discuss a difficult diagnosis. Perhaps you needed to confront
someone who was not following hospital rules, or who was not adhering to medical advice (and
was being confrontational about it). Perhaps this person was openly abusive to you or another
member of your team.
A. First, think of a time when this conversation went well. Describe the scenario and what you
did and said that led to the good outcome.
B. Now, think of a time when you had a difficult or challenging conversation and it did not go
well. Using what you have learned from the readings, what might you have done differently.
Write a script for yourself – because practicing the actual words we might say in a given
situation is the only way we will learn to say these words when the time arises.
C. It is important to realize that even if we say or do the right things, and respond with
compassion and caring, not all difficult or challenging conversations will have what we think
of as a good outcome. Nor will all conflicts be resolved. What might you say to a patient or
their family if resolution does not happen? What other resources might you tap into to help
resolve the issue?
2. In class I will ask you to practice difficult and challenging conversations. The following are
scenarios you can choose to role play. After reading through the scenarios, indicate which one
you will practice, and write a possible response to each of the following communication steps:
A. Use Skill Set One (Chapter 3 in Chou & Cooley) to
1. Establish rapport
2. Elicit the list of all concerns
3. Negotiate a shared agenda
B. Use ART (Ask, Respond, Tell) to check understanding, to express empathy or to deliver
information:
1. Ask the patient or support person their perspective on the situation.
2. How would you respond to them with active listening?.
3. Tell them your perspective.
C. Now pivot to teaching, to explaining what you want the patient or their support person to
do differently.
1. What type of open-ended questions might be appropriate for this situation?
2. How would you elicit their ideas and expectations?
3. What types of emotions might the event trigger in the patient or their support person?
How would you respond to these using PEARLS?
4. Imagine they are being cooperative. What type of teaching is needed?

This week I want you to consider your role in policy change and advocacy. We have been talking about this over the past few weeks; however, how do you see your role as a bedside nurse (as many of your are not yet advanced practice nurses)? I know for a long time I did not see the point in becoming involved in policy making as a bedside nurse but as I started to practice more and more I realized as the old adage goes, “don’t complain if you don’t have a plan to fix it!!!” That very much applies, as you practice for a few years you recognize and start to question or see how a change is needed and how something might be done a different and/or a better way. Yet, to affect any ‘real’ change you need to get involved in how a policy or practice is currently in place.
I suppose this week I want each of you to consider how policy truly influences what you do as a nurse both from the perspective of where you are now and where you will potentially be as an advanced practice nurse. What role can you realistically play as an advocate for policy change, what does this process really look like not as an idea but as a reality. On that note, read the assigned articles provided this week and discuss the following
Consider the process of policy change from the perspective of both the bedside nurse and the advanced practice nurse. Consider your readings from this week that discuss what the healthcare workforce looks like, the role of information technology in healthcare and the role of board members and managers within the context of your current workplace environment. Choose a policy or practice within your current workplace environment (or if not actively practicing consider one that you have encountered while practicing at the bedside) that you wish to advocate for policy change. Describe your rationale for choosing this specific issue using supporting evidence as to why a change needs or should be made (make sure to support this response with evidence and your own insight as a practicing nurse)
Using the Framework for Planned Policy Change (as outlined in the Anders et al., 2021article) & the Health Policy Toolkit (outlined in the Kostas-Polstonet al, 2014 article; and I also included a section on Legislative Resources in this week’s Blackboard Module that can assist with this process) describe the specific steps and/or processes (in detail) you would use to make a policy change to your chosen topic at both the local (hospital or organizational level) and also at the state or national level (note that not all the steps in the Framework or the Toolkit will apply so focus on those steps and processes that do apply to your topic).
Describe how the process of this selected policy change would differ from the perspective of the bedside nurse versus that of an advanced practice nurse. Essentially this question is asking you to consider the difference in policy change engagement from the perspective of the bedside nurse compared to that of an advanced practice nurse level?
Discuss barriers and reasons nurses (at least 2 barriers) fail to participate in policy change and describe how these barriers can be addressed.
What are your personal thoughts about being involved in policy change whether at a local or state/national level. Have you been involved in policy change at any level? Why or why not? If so, describe your involvement. If not, describe why you may not have personally been involved in policy change up to this point. This is genuinely an opportunity
